About Assessment Request

Most related services require assessments to determine eligibility. Even if your child already has an IEP, you might believe that he needs additional services. You have a right to ask for the assessments needed to determine eligibility for those additional services. It is very important that the request for assessments be submitted in writing and that your request be signed and dated. This will start the timelines, meaning that the school district must complete the assessments, write reports, and hold an IEP meeting within the timelines given in state and federal special education laws.

Below are some possible assessment areas. You will NOT ask for all of these assessments. You will need to decide which assessments your child needs. Some of the assessments will be offered by the school district. To make sure that all areas of suspected disability are assessed, you may need to identify possible needs based on what you know about your child.   • Health and development
  • Audiology
  • Intellectual development
  • Academic achievement
  • Auditory and visual processing
  • Social and emotional status
  • Behavior
  • Speech and language (including pragmatics)
  • Occupational therapy (including self-regulation/sensory integration)
  • Physical Therapy
  • Orientation and mobility
  • Adapted physical education
  • Travel training (especially for children with significant cognitive disabilities and any others that require this instruction)
  • Transition/vocational
  • Recreational
  • Medical
  • Interpretive (for students that are deaf or hard of hearing or deaf-blind.)
  • Assistive technology
  • Vision therapy

You will need to put your name and address and other information to make the letter particular to your child’s needs. The content of the letter is written in first person plural or as if it is coming from two parents. Change the plural pronouns (we, us, our) to singular pronouns (I, me, my) if the letter is coming from one parent.
